SUMMARY

As a Registered Nurse (RN), you will maintain quality patient care through consistent standards of nursing practice in collaboration with the multidisciplinary team. You will utilize the nursing process: assess, diagnose, plan, implement, and evaluate to execute appropriate individual patient care. The RN is instrumental in providing an exceptional experience for patients and families.

ElderONE

An affiliate of Rochester Regional Health, a Program of All-Inclusive Care for the Elderly (PACE), servicing Monroe, Ontario, and Wayne counties. ElderONE is an innovative alternative to long-term nursing home placement. This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.

LOCATION: 2066 Hudson Ave, Rochester, NY 14617

DEPARTMENT: ElderONE

SCHEDULE: Monday-Friday, Day Shift, limited Saturday hybrid rotation

ATTRIBUTES
  1. Registered Nurse license in New York State required
  2. Diploma or Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred

RESPONSIBILITIES
  1. Patient Care & Service: Promote and restore patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; performing various treatment procedures; providing physical, educational, and emotional support to patients, friends, and families; supervising assigned team members.
  2. Planning & Communication: Develop and document individualized care plans customized for each patient's unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans, and progress.
  3. Electronic Health Record (EHR) Management: Demonstrate proficient use of an EHR, including accurate patient and provider documentation and communication.
  4. Compliance: Adhere to required department and system protocols, regulations (local, state, federal), and education requirements.

EDUCATION:
  1. AS: Nursing (Required)
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing, or squatting.
