Job Summary:
Provides professional social work services to individual patients and their families, including social assessment and referral to community resources. This position is an integral part of the care team and provides case management services for appropriate patients identified in need of services. The Social Worker is responsible for the timely and accurate delivery of social work services upon referral of patients by providers, assuring that services are delivered in a courteous and professional manner and in accordance with applicable policies and procedures, rules, regulations, and prevailing law. The Social Worker will maintain effective relationships with other community agencies to support cross-referral of patients. In matters pertaining to social work, this position serves as "internal consultant" to providers and staff within the practice.
Primary Job Responsibilities:
Referral Management : Initiates or accepts referrals from staff for patient needs assessment, determines eligibility for resource assistance, and makes resource referrals. Provides assistance with referral for durable medical equipment, safe cribs, and car seats.
Case management : Provides case management services. Coordinates case management with nursing staff as appropriate. Provides social assessment, care plan, and systematic proactive follow-up. Provides consultative services to providers and other members of the staff.
Program Development and Evaluation : Identifies new programs or services and makes recommendations to management. Evaluates the effectiveness of existing and new programs/systems from the standpoint of the social needs of the population served and their impact on social work functions.
Community Network : Establishes/maintains positive, ongoing working relationships with other community agencies.
Outreach Activities : Staffs and/or coordinates staffing of community events, health fairs, etc. in which the center participates. Makes public relations or informational presentations to community agencies, businesses, service and other organizations. Develops a means of documenting outreach activities to support a historical archive, and for use in budget development and planning. Creates and maintains promotional materials and adequate inventory.
Community Resources : Establishes and maintains effective network of community resources for cross-referral of patients. Develops and maintains a current resource directory for primary practice site and organizational expansion sites. Develops and maintains a current resource directory for a wide range of support services to complement and support the primary care and psychiatric care provided through the center.
Integration : Promotes integration of social work services into primary care model through identification of patient-specific interventions and new programs, and serves as "internal consultant" to providers and staff through evaluation of potential programs and their impact on social work.
Minimum Qualifications:
LCSW required. Minimum two years' experience in medical social work in an ambulatory setting with at-risk patients preferred. Knowledge of available community resources and the ability to evaluate resources and make appropriate referrals. Knowledge of health-related financial assistance, including Medicare and Medicaid.
Salary Range: $33.43 - $54.89
Pay Grade: 34
