The Supported Living Coordinator serves as the liaison between the daily operations of our Supported Living Homes, which provide services for individuals with intellectual disabilities, and our Residential Program Manager. This position is responsible for the overall daily functions, ensuring staff coverage, and documentation of the home they are assigned to, supporting and ensuring the physical, emotional, and psychological needs of the individuals in our care, maintaining the safety of our sites, and serving as the in-house expert for compliance, documentation and individual care.
Essential Job Functions:
  • Conducts bi-weekly household inspections, and routine reviews to ensure cleanliness and safety of the Supported Living homes.
  • Advocate for each individual to ensure that all required documentation and assessments are completed annually or within designated time frame and provides information and input for completion.
  • Maintains open communication with Supported Living Program Management pertaining, but not limited to, individuals in our care, health and safety concerns, compliance issues, staff performance, etc.
  • Collaborates with Director of Community Health to develop and monitor menus and ensures all nutritional requirements are met, calorie counts, food consistency, and other dietary restrictions and health maintenance are as ordered by the individuals PCP.
  • Serves as a member of the Interdisciplinary Team, assisting in the development of a comprehensive treatment plan which addresses the individual's needs and promotes their maximum independent functioning.
  • Assists with meeting the needs of all individuals in a supported living capacity, such as personal finances, shopping, meal preparation, and other activities of daily living, which include bathing, toileting, dressing, housekeeping, laundry, etc.
  • Ensure that each individual is consistently provided supervision level(s) as designated on the annual development assessment.
  • Collaborates with direct care staff on providing the individuals in our care with fulfilling leisure, recreational, and social activities as well as community outings in accordance with the individuals likes and dislikes.
  • Assists with training of new and current staff that work in the homes to ensure that each employee has all skills and knowledge necessary to provide quality care to the individuals in the homes.
  • Is the first responder "on-call" 24 hours/day, Monday through Friday for supported living home and then part of an on-call rotation for weekend hours.

Requirements
  • A minimum of 2 years' experience working in direct care; experience working with individuals diagnosed with an intellectual disability, required.
  • Experience working in a group home, preferred.
  • High School Diploma/GED, required.
  • Supervisory experience, demonstrated leadership qualities, proven initiative, motivation, and follow-through in a work setting, strongly preferred.
  • Ability to lift, turn, and move objects of up to and including 30 pounds. If over 30 pounds, ability to assist in a two or multi-person lift.
  • Physically capable of participation in physical management of individuals. Ability to perform transfers of individuals (i.e. from wheelchair to bed, toilet or positioning equipment) and assist in toileting/changing individuals who are physically unable to do so themselves or who require some level of assistance. Involves physical assistance ranging from stabilizing individual to doing a Multi-person lift of an individual weighing 100 plus pounds.
  • Ability to stand and move about freely for extended periods of time.
  • Ability to travel at a moderate to heavy degree in car or large passenger van/minibus. Employee may be exposed to weather conditions prevalent at the time.
  • Valid driver's license. Must maintain driver's license during employment.
  • Use of personal vehicle for transportation of individuals while working in the community
