Selected candidate should possess strong communication, organizational skills and heightened attention to detail. Finds patients assigned to case load and enrolls them or gets out of client search in acceptable timeframe, and participates in staff meetings.Clinical: Contacts with families/collateral contacts are strengths-based. Services are delivered in ways that are responsive to family needs and are medically necessary. Interventions utilize naturally occurring resources when possible and are focused on assisting families in their efforts to maintain patient in least restrictive environment.Judgment: Demonstrates sound ethical judgment, models good self-management skills, and appropriate professional boundaries consistent with agency values, especially during times of crisis.Documentation: All documentation is completed in a timely manner, is thorough, complete, clinically appropriate, and complies with agency standards. Documentation demonstrates medical necessity and accurately reflects interventions and family/patient progress toward goals. HAPs are completed on time, appropriate referrals made, and hospital or emergency room alerts are responded to quickly and documented in Care Manager.Leadership: Demonstrate gradual but continual professional growth (knowledge and skills). Models respective, supportive, and professional conduct in all work environments. Demonstrates an awareness of how behaviors and speech impact the whole team.Coordination: Works collaboratively with both agency peers and partnering organizations while building a professional network that assists all families being served. Ensures that all case related paperwork is completed in a timely manner, readily accessible/available to family/treatment team.Financial: Payment sources and billing are reviewed and addressed regularly. Understands and supports business model needed to financially support treatment providers and family needs.
