Job Summary

We are seeking a Care Coordinator to join our team. In this role, you will work collaboratively with patients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, reliable, trusted and knowledgeable about healthcare practices.

Responsibilities

Care Planning and Coordination

Collaborate with physicians, patients, families, and healthcare staff

Develop and review individualized care plans, ensuring they are followed

Create patient goals and monitor progress

Coordinate care internally and with external providers

Perform intakes and update care plans as needed

Conduct home supervisory visits when required

Staff Management and Development

On Board and train new staff members

Provide ongoing training and management for caregivers

Scheduling and Operations

Coordinate with schedulers to meet client needs

Receive and triage inbound calls

Participate in rotating on-call duties

Compliance and Documentation

Ensure compliance with all company policies, state, and federal regulations

Manage client and caregiver files and credentials in accordance with policies and regulations

Create and revise policies and procedures for direct reports

Client Relations

Handle client inquiries and concerns

Conduct regular check-ins with clients and their families

Quality Assurance

Monitor the quality of care provided and implement improvements as needed

Update and train staff on care plans when discrepancies are identified

Additional Responsibilities

Assist with agency operations as needed

Adapt to changing healthcare regulations and best practices

Qualifications

Previous experience as a Care Coordinator or in a similar position is preferred

Previous experience in home care is required

Strong problem-solving and organizational skills

Ability to manage multiple projects or tasks and prioritize appropriately

Ability to work in fast-paced situations and make sound decisions quickly

Excellent interpersonal skills and high level of compassion

Strong verbal and written communication skills
