JOB PURPOSE: The Preferred Communities (PC) Refugee and Entrant Assistance (REA) program supports vulnerable refugee and ORR-eligible populations who need assistance as they integrate into the United States, such as those with serious medical or psychological conditions, single parent/guardians, survivors of domestic violence or gender-based violence, or those from vulnerable age groups with minimal family support.The Preferred Communities Case Manager will provide services to clients in the Cuban/Haitian Initial Services (CHIS) and the Intensive Case Management (ICM) programs. The Preferred Communities Case Manager will collaborate with clients through an inclusive, strengths-based, and client-centric approach to develop and achieve personalized goals. In addition, the Case Manager will work with service providers to enhance their capacity to serve vulnerable populations.

Qualifications

ESSENTIAL JOB RESPONSIBILITIES:

  • Receive comprehensive training on the Preferred Communities program CHIS and ICM service components, Matching Grant (MG), and other programs for which Cuban and Haitian humanitarian parolee and entrant clients (CHHPs) are eligible.
  • Build and maintain a caseload of 45 clients in the first fiscal year of the program (29 CHES clients and 16 ICM clients).
  • Conduct outreach to identify eligible program clients, support client enrollment, and conduct initial program screening.
  • Conduct and document client check-ins, home visits, screenings, assessments, and referrals in accordance with the Preferred Communities program plan and service timeline.
  • Collaborate with clients to identify strengths, challenges, and needs in the resettlement process, and to create self-sufficiency service plans through an inclusive, strengths-based, and client-centric approach.
  • Track and document client progress in accessing and maintaining services for families and children, social adjustment, employment, basic needs, English language acquisition, knowledge of U.S. culture/institutions, and assistance services.
  • Assist with applications for Social Security cards, Employment Authorization Documents, driver's licenses, school enrollment, among other public benefits, as appropriate.
  • Provide skill-development and information for the client to acclimate successfully to their community and meet and maintain their goals.
  • Ensure all case files, reports, and other documentation are kept up to date and maintain compliance with program expectations.
  • Assist with the preparation of program reports.
  • Collaborate with volunteers and program partners, including local service providers, to ensure clients receive needed services, seek collaborative opportunities to improve service delivery, and to expand the range of available referrals.

ESSENTIAL QUALIFICATIONS:

  • Understanding of available community resources that impact and/or benefit the targeted client needs.
  • Knowledge of basic client management procedures for determining eligibility, assessing needs, identifying resources, making referrals, following up, and documenting client interactions and proactively supporting client's pursuit of goals.
  • Fluency in both English and a second language (especially Spanish, Haitian Creole, and/or French) is strongly preferred.

EDUCATION: Bachelor's degree in Social Work or related field

EXPERIENCE: 1-2 years of relevant professional experience.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: Driver's License with clear record.

VEHICLE: Valid drivers' license with access to reliable transportation

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Ability to work outside of the 9-5 schedule, as needed. Ability to work with a high degree of autonomy and accountability while being a team player.

ESSENTIAL KNOWLEDGE, SKILLS, and ABILITIES:

COMMUNICATION: Ability to communicate clearly and effectively via oral or written means. Ability to present a friendly and positive demeanor to parents, school personnel, licensing, staff and the general public. Ability to make oral presentations.

COMPOSURE: Ability to remain calm and maintain self-control in the midst of difficult circumstances and emergencies. Ability to respond in a professional manner in all situations.

DECISION MAKING: Ability to set work priorities and to evaluate and create solutions to work-related problems. Ability to think analytically and evaluate the impact of decisions.

ORGANIZATION: Excellent planning and organizational skills; ability to prioritize, manage time efficiently in a fast-paced work environment and remain flexible in an ever-changing landscape for refugee issues.

TRAINING REQUIREMENTS:

Orientation: MS-OST, New Employee Orientation, program-specific training

MACHINES, TOOLS, EQUIPMENT, WORK AIDS:

Multi-line telephone, computer, ten key calculator, printers, copiers, faxes. Knowledge of Microsoft 365 platform, including SharePoint and MS Teams, preferred; proficiency in Microsoft Excel, PowerPoint and Word is essential.

PHYSICAL REQUIREMENTS:

Constant - Ability to communicate with management, staff and clients over the telephone and in person.

Constant - Ability to input data into computer, make calculations, complete paperwork, and produce reports.

Constant - Ability to sit for prolonged periods of time.

Occasional - Ability to move to different offices throughout the building.

WORKING CONDITIONS:

Environment: Hybrid work from home model with a typical office environment. Must be able to travel to and attend home visits in various communities.
