Transition Coordinators will be responsible for assisting program participants to enhance their daily living and employability skills through the implementation of structured activities designed to promote a successful transition to the next phase of employment supports. The Transition Specialist will provide individualized training consistent with the participant’s service plan. The Transition Specialist will support participants in activities that take place in community-integrated settings, including community-based job sampling and volunteering. The Transition Specialist will promote an environment that fosters the acquisition of skills and encourages positive social behavior, greater independence, and personal choice.
