Join our team of compassionate Social Workers dedicated to empowering patients and their families to achieve optimal psychological, emotional, spiritual, social, and physical well-being. Our Social Workers identify and foster coping skills and strengths within patients and families. They play a crucial role within the interdisciplinary team, addressing psychosocial stressors and collaborating to enhance the quality of end-of-life care for patients and their loved ones.
- Conduct initial psychosocial evaluations within required time limits, following hospice regulations and Conditions of Participation.
- Provide guidance on advanced care planning, including directives, DNRs, DPOAs, and funeral planning, as per patient and family wishes.
- Assist patients and families in understanding and following medical recommendations for optimal social and health adjustment.
- Utilize available resources to support patients and families living with illness limitations.
- Effectively manage daily tasks, balancing priorities and completing duties independently.
The schedule for this position entails Monday through Friday day shifts. On-call duties, which may include after-hours and weekend work, will be rotated based on location.

Requirements
QUALIFICATIONS AND LICENSURE:
- Graduate of an accredited school in social work, psychology, or other related field.
- Licensed Social Worker licensure with no encumbrance, preferred
- Compassionate and empathetic nature, with a genuine desire to provide comfort and support to patients and families/loved ones during a challenging time.
- Valid driver's license with current automobile insurance
- Reliable vehicle
