**Job Description:**
- Ensure prescreen is filled out and attached when submitting a candidate
- Attach resume and license verification to submission
- Candidates not providing these documents will be declined
- Conduct a COVID-19 test 72 hours prior to the start date with results available 48 hours before the start date
- Complete the credentialing checklist before submission, including:
- State License Verification
- Resume
- Three references
- County Criminal check (7 years for all counties lived and worked)
- Federal Criminal check (7 years or more)
- Multi-State Criminal Search (7 years or more)
- Nationwide Sex Offender Search (7 years or more)
- HHS/OIG Search
- GSA Search
- Social Security Number Trace
- 2-Step PPD or Quantiferon or proof of positive PPD and Chest X-Ray
- SAM Search
- Washington State Background check with fingerprints conducted by the facility
- Position urgency is normal
