The Care Navigator will be responsible for maintaining an organized system of scanned and filed charts, and will interact with Nutrition staff to ensure client and service level information is captured and reported accurately.Essential Job Functions and ResponsibilitiesThe following duties are mandatory requirements of the job:Conduct data entry services into the TREAT and eShare in accordance with departmental and funder policies.Document all program activities and referral related services into the TREAT system and other systems as needed.Run data entry reports to verify accurately entered test information.Identify and troubleshoot potential data entry problems.Assist with special projects and perform other related duties as assigned by supervisor.Assist with translation of client appointments and program materials as neededAssemble and distribute food bags at weekly food distributionsRequirementsEducationRequired: bilingual in Spanish and EnglishCultural and linguistic competence for the target populationHigh School Diploma or its educational equivalent requiredSpecial Skills and KnowledgeIn addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:Experience in data entry/data managementDetail orientedExcellent organizational skillsAbility to maintain focus on repetitive tasksExcellent written, oral and interpersonal skillsExcellent computer skills, including MS Word and ExcelSalary Description$48,000 per year
