Job Requirements / Description
What will you do?
  • Serves as a Community Resource Program Coordinator in assigned Developmental Center (DC) in the Department of Developmental Disabilities (DODD).
  • Provides assistance to the DCs in completing person centered Discharge & Transition Plans that meet each person's individual needs.
  • Provides guardian/family education regarding community placement options.
  • Creates strong link & serves as a liaison between centers, community providers & County Boards to develop meaningful options for transitions.
  • Assists in breaking down system barriers to providing services needed to ensure the growth & development of each person transitioning in to a community based option.
  • Ensures community provider staff are trained in effective delivery of training techniques/methods to serve individuals being discharged from DCs.
  • Ensures appropriate community resources are in place prior to discharge.
  • Gives guidance & direction to staff in community homes contracting with DODD to serve people from DCs.
  • Consults with the individual's family member &/or guardians as well as their community providers to make sure the goals of individuals are being met.
  • conducts &/or participates in Discharge Reviews (e.g., meets with individuals for in-person evaluations, reviews data analysis, ensures proper monitoring & follow-up procedures are in place & provides reports on the individual's progress.
  • responsible for Quality Assurance Review & follow-up when required (e.g., program assessment, survey preparedness, review of Major Unusual Incident (MUI) reports, licensure renewal preparation & plan of correction review & monitoring.
  • responsible for the Individual Assessment Form (IAF) exception review process (e.g., completing &/or reviewing assessments, exceptions & reconsiderations).
  • Provides regional technical assistance to groups such as the Private Provider Teams.
  • Serves a liaison between DODD & the County Boards of DD & community providers on matters concerning the Community Resource Program.
  • Presents training to stakeholders on the Trauma Informed Care Initiative; consults with others on cases as assigned.
  • Completes the Ohio Developmental Disability profile (ODDP) as assigned.
  • Works on Support Intensity Scale (SIS) as assigned.
  • Completes other duties as assigned (e.g., reports, special projects).
**This position is hybrid under the agency's current telework agreement.**

Qualifications
4 yrs. administrative exp. commensurate with mental health program area to be assigned per approved Position Description on file.
-Or completion of associates core program in mental health, social work or comparable field (e.g. child & family community services, special education, social or behavioral sciences); 12 mos. trg. or 12 mos. exp. in state &/or federal regulations & standards governing care, custody & programming of mental health consumers/residents; 12 mos. trg. or 12 mos. exp. in mental health programmatic project management.
-Or completion of undergraduate core program in mental health, social work or comparable field (e.g. child & family community services, special education, social or behavioral sciences); 12 mos. trg. in employee training & development (e.g. provides work direction & training); 12 mos. trg. or 12 mos. exp. in state &/or federal regulations & standards governing care, custody & programming of mental health consumers/residents.
-Or 6 mos. exp. as Mental Health Administrator 1, 65221.
-Or equivalent of Minimum Class Qualifications for Employment noted above.
