CARE COORDINATOR-SBHI

The Care Coordinator functions as a member of a multi-disciplinary care team providing services for persons with an 
alcohol and/or other drug Substance Abuse Disorder diagnosis. The primary responsibilities of the care coordinator 
include: 1) Support adherence to the patient’s treatment and recovery plan; 2) Manage successful care transitions; 
3) Engagement in care; 4) Services may be delivered by phone, telehealth, face-to-face at the office, in the home, or 
in the community. This position requires expertise in transition planning and process, resource allocation, team 
management and communication skills.
