The Patient Navigator enhances the patient and provider experience by appropriately coordinating the care of new patients across multiple specialties. The NPC will guarantee that new patients are prepared for their initial visit by providing them with education packets and direction while also ensuring the patient has provided the office with all information (records, imaging, etc.) pertinent to their visits. There is a strong emphasis on customer service, communication and decision making.
- The Patient Navigator is the main point of contact for new patients in the Department(s) and is expected to convey emphathy and competence in coordination of the patientsrsquo; care
- Responds to requests for new appointments within 24 hours
- In collaboration with the clinical team, the NPC will follow the new patient triage workflow to ensure that new patients are cared for by the appropriate provider
- Frequently meets with clinical team to ensure triage workflow is up to date and effective
- Schedules new patient appointments and actively manages new patient slots to ensure access and appropriate usage
- Responsible for new patient chart prep (collation of imaging, test results, pathology results, etc.) to ensure a robust and successful new patient visit
- In cases where patients do not have all information needed for visit, the NPC will assist the patient in further coordination to be fully prepared for a new patient visit
- Chart prep finalized 72 hours prior to new patient visit
- Assists in developing new patient materials
- Develops and cultivates relationship with care teams and managers
- Expert knowledge of all systems that are used by NPC (Epic, Outlook, Excel, Word, etc.)
High School Diploma or GED required
Bachelors Degree preferred
