Shift : Days, 8a-4:30p, 8x5, 40hrs per week
Length: 13 Weeks

Requirements:

• IL license
• 3-5 years in hospital setting, care coordination, discharge planning
• BLS
• Prefer ACM or CCM

Needs Active IL license

What level of experience is required?: 3-5 years in hospital setting, care coordination, discharge planning
