As the Care Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through:
Attempt to make contact with referrals or assigned members promptly, within program guidelines timeframes
Use written and electronic tools to fully assess members’ medical, mental health, substance abuse and psychosocial needs
Develop a care plan through case conferencing/consultation with all member’s providers, to guide all service delivery and achievement of desired program outcomes
Represent the agency in keeping with agency mission and standards of professionalism, code of conduct and ethics.
Observe and enforce Confidentiality Laws governed by the Health Home Consent form.
Incorporate the Trans theoretical Model of Behavior Change and Motivational Interviewing into all aspects of service delivery.
Care Coordinator Minimum Salary: $19.48
Care Coordinator Maximum Salary: $23.07
Qualifications:
Bachelor’s degree
Experience working with a diverse population
