Overview

Mercy Medical Group (MMG) is a multi-specialty group with more than 500 healthcare providers in the Sacramento region, is looking to add a Nurse Practitioner to its busy Geriatrics - House Calls team. A background in homehealth or hospice is a plus. You make your own schedule seeing up to five homebound (5) patients per day. Compensation range depending on experience: $134,000 - $176,000 based on experience

MMG clinicians provide services to patients at Dignity Health Medical Foundation locations throughout Sacramento. MMG is aligned with Dignity Health, operating on a medical foundation model. Responsibilities

This position would involve visiting complex patients in their homes, assisted living facilities and board and care facilities in support of PCP's managing multiple medical conditions, to prevent unnecessary ER visits or hospital admissions. The successful candidate will work with a complete interdisciplinary team including Geriatricians, social workers, pharmacy and case management.


Qualifications
  • A valid and unrestricted license to practice in the State of California
  • Nationally recognized certification (as determined by Medicare)
  • Completion of state mandated educational courses


Pay Range
$64.43 - $85.06 /hour
