Embark Outreach Case Manager
Join a team dedicated to making a positive impact on the lives of vulnerable individuals. As an Embark Outreach Case Manager, you'll work directly with unhoused and at-risk individuals utilizing the Embark transit system. Your role will involve building relationships, connecting them to essential resources, and providing support to help them achieve stability.

Monday - Friday
8:30am - 5:00pm
Join a team dedicated to making a positive impact on the lives of vulnerable individuals. As an Embark Outreach Case Manager, you'll work directly with unhoused and at-risk individuals utilizing the Embark transit system. Your role will involve building relationships, connecting them to essential resources, and providing support to help them achieve stability.
Key Responsibilities:
- Develop and maintain relationships with individuals experiencing homelessness, mental health challenges, or social concerns.
- Collaborate with Embark leadership, community partners, and MHAOK staff.
- Connect individuals with housing, mental health treatment, recovery programs, employment services, and other necessary resources.
- Provide transportation to clients when needed.
- Complete necessary documentation and data collection.
- Contribute to program evaluation and development.
- Must be in active recovery for a minimum of 2 years.
- Bachelor's degree in human relations, behavioral health, social work, or a related field.
- Experience working with vulnerable populations.
- Excellent communication and organizational skills.
- Ability to work effectively in a multidisciplinary team.
- Strong technology skills.
- Flexibility and adaptability.
- Valid Oklahoma Driver's License and clean driving record.
