Hospice Medical Social Worker

We are seeking a compassionate Hospice Medical Social Worker to join our team. In this role, you will provide psychosocial evaluations and ongoing support to patients and their families during their end-of-life journey. Qualifications:

• A strong desire to serve patients and families
• Education: MS degree in social work from an accredited school
• Licensure: Current state social work license
• Experience: 2+ years in clinical work, preferably in healthcare or hospice
• Requirements: Reliable transportation, ability to lift at least 25lbs
