**Certifications:** Basic Life Support for Healthcare Providers (CPR and AED, may be obtained within 1 year of hire)
**Must-Have:**
- Pharm.D. degree and licensure as a registered pharmacist
- Strong, recent inpatient pharmacy experience
**Job Description:**
- Review all prescriptions for appropriateness of therapy, legality, completeness, contraindications, interactions, allergies, adverse drug reactions, and lab value abnormalities.
- Provide clinical consultations, clarify, and resolve concerns or questions with prescribers prior to dispensing.
- Document all clinical activities and interventions accurately.
- Enter prescriptions into pharmacy software and verify finished prescription orders in a timely manner.
- Advise patients clearly and accurately on their medications and appropriate outcomes and verify patient understanding.
- Communicate regularly with patients and healthcare providers to resolve problems and provide optimal care.
- Perform chart reviews for prescription refills and medication therapy management.
- Provide drug information, drug therapy consultation, and staff education related to drug therapy.
- Direct pharmacy support personnel and verify daily activities of pharmacy technicians.
- Participate in clinical and administrative activities, ensuring quality control and improvement within the department.
- Attend meetings and participate on committees as requested.
- Provide health promotion and disease prevention activities related to drug use and preventative drug therapy.
- Must be able to work independently, prioritize work, make critical decisions, and respond quickly to emergency requests.
- Maintain proficiency in reviewing patient medical histories, lab results, and medication orders.
- Be knowledgeable of state and federal laws governing pharmacy practice and provide age-appropriate care to patients.
- Communicate effectively both orally and in writing, and possess math skills for accurate drug dosage calculations.
