HomeCare Social Worker (LMSW, LCSW)

Job Responsibilities:

• Collaborate with interdisciplinary team to identify patients in need of Social Work Services
• Complete comprehensive in-home psychosocial evaluations
• Implement short-term interventions to address barriers to care and promote health and well-being
• Provide clinical treatment services such as triage, crisis intervention, and therapy
• Educate patients, families, and community on disease prevention and health maintenance
• Facilitate team-based care and collaboration
• Identify community-based resources for patients and families

Qualifications:

• 2-3 years health care experience, inclusive of acute and community health experience
• LMSW required
• Familiarity with evidence-based assessment tools and community resources
• Excellent communication and organization skills
• Strong computer skills including Microsoft Office

Mental Health First Aid Certification, QPR Training, and Movement Disorder Training required for new clinicians
