Responsibilities:

• Meet clients and families in their home setting.
• Develop a plan of care including nursing and support services that meet the client's needs.
• Determine what caregiver team members are appropriate to meet the client's needs and manage that team ongoing.
• Available to client and team on a daily basis to ensure services are provided timely and in full compliance with the client's stated desires.

Requirements:

• Licensed Registered Nurse. BSN preferred.
