Housing Case Manager

We currently have 3 Case Managers to support our properties. Offices located at the Nakoosa Office (1) or Theirer Road (2).
SUMMARY OF PRIMARY FUNCTION
The Housing Case Manager provides individualized support services to residents of Porchlight housing, with the primary goal of promoting housing stability. This position is responsible for orienting and enrolling new residents, making referrals to appropriate community resources, completing thorough and timely documentation, and acting as an advocate and liaison for housing residents.
Full-time, 8:30am - 4:30pm, Monday - Friday.
Starting pay is $20.00/hour with a COLA raise at end of year.
ESSENTIAL RESPONSIBILITIES/DUTIES
- Works with the Director of Supportive Services to receive housing referrals from Coordinated Entry, or adhere to an alternate tenant selection plan, depending on unit requirements. Meets prospective residents to show open units as requested, documents interactions.
- Supports prospective residents during the housing application process. Conducts interviews to review program services and expectations, and to assist with any necessary forms or releases.
- Enters timely and complete enrollment and exit documentation for all residents in the Homeless Management Information System (HMIS) database.
- Works with residents to develop comprehensive, client-driven case plans that identify both short- and long-term goals, utilizing housing first, harm reduction, and trauma-informed care best practices.
- Empowers residents to be involved in their own planning and goal setting, refers them to appropriate resources to meet their goals, evaluates and adjusts case plans as needed.
- Provides support and monitors progress towards goals in regularly scheduled bi-weekly or monthly meetings with residents, including home visits as appropriate. If a resident declines case management, services are still actively offered on a quarterly basis throughout tenancy.
- Maintains accurate, timely documentation of case notes, assessments, and updates in HMIS.
- Helps eliminate barriers to housing stability, including assisting residents with budgeting/money management, understanding and complying with their lease, and resolving issues as needed.
- Organizes work, sets priorities, and manages time effectively in order to meet the needs of residents and meet documentation expectations.
- Mediates interpersonal conflicts between residents, de-escalates situations whenever possible.
- Completes thorough and objective incident reports as necessary. Reports critical incidents immediately to the Director of Supportive Services and/or Director of Housing. Assists residents in composing and submitting incident reports to their landlord as appropriate.
- Notifies proper agencies of such issues as suicidal/homicidal ideation, significant changes in behavior indicating instability, criminal activity, and probation/parole violations, as appropriate.
- Collaborates with other service providers working with residents, attends team meetings as needed.
- Works with the Porchlight housing department staff and Director of Supportive Services to ensure that housing units are habitable, maintenance orders are submitted and completed, and potential capital projects are identified at Porchlight properties.
- Works with the Director of Supportive Services to collect resident feedback, and makes recommendations for service improvements.
- Attends biweekly supervision meetings with the Director of Supportive Services, participates in regular case management staff meetings.
- Represents Porchlight on a Homeless Services Consortium of Dane County (HSC) committee or other community work group(s) as recommended by the Director of Supportive Services.
- Performs other duties and takes on other responsibilities as assigned by the Director of Supportive Services or Executive Director.

QUALIFICATIONS
- Bachelor's Degree in Social Work or related field desired, or equivalent knowledge and experience.
- Experience working in a nonprofit social service agency.
- Strong crisis management skills.
- Desire to work with a diverse population of individuals with different strengths and barriers. Sincere respect for individuals experiencing or who have experienced homelessness.
- Ability to motivate and obtain willing cooperation from others.
- Team orientation with respect for others. High ethical standards.
- Proficiency in Microsoft Word, Excel, Outlook, and Teams. Ability to adapt to new technology.
- Driver's license, insured personal vehicle.
