Gable Pines is recruiting for a hospitality focused Health & Wellness Navigator to join our team!
Here are a few of the daily responsibilities of a Health & Wellness Navigator:

• Assists residents with the transition with the continuum of care services to the appropriate levels of care offered within the community.
• Coordinates new resident orientation.
• Coordinates communication with physicians, families, and appropriate staff regarding resident's status.

Here are a few of the qualifications we need you to have:

• Three to five years of experience working in the Senior Living Space, Independent Living, Assisted Living, and Memory Care or other health care setting with strong organizational skills, management experience with care coordination and care management with an interdisciplinary approach to care preferred
• Graduate of an accredited college with a RN, or bachelor's degree in social services, geriatrics, nursing, and or related senior living services preferred
• Effective organizational skills including delegation, managing multiple priorities, time management

Pay: Starting at 65,000/year
