Patient Care Advocate
HealthCare Support is actively seeking a Patient Care Advocate to fill an opening with a Fortune 50 Managed Care Company in Saginaw, MI .
Daily Responsibilities for Patient Care Advocate:
  • The Patient Care Advocate will be working with members to ensure care gaps are closed, barriers to care are removed, and improve member and provider experiences.
  • Serves to collaborate with providers in the field, to improve HEDIS measures and provides education for HEDIS measures and coding.
  • Supports the implementation of quality improvement interventions and audits in relation to plan providers.
  • Assists in resolving deficiencies impacting plan compliance to meeting State and Federal standards for HEDIS.
  • Conducts telephonic outreach, while embedded in the providers' offices, to members who are identified as needing preventive services in support of quality initiatives and regulatory/contractual requirements.
  • Provides education to members regarding the care gaps they have when in the providers office for medical appointments.
  • Schedules doctor appointments on behalf of the practitioner and assists member with wraparound services such as arranging transportation, connecting them with community-based resources and other affinity programs as available.
  • Maintains confidentiality of business and protected health information.
  • This will be a Contract role from 11/11/2024 to around 12/31/2024 (potential to extend)
  • Shift: M-F 8:00-5:00 w/ 1 hour lunch or 8:00-4:30 w/ 30 min lunch
  • Location: Must live within Saginaw/Genesee counties (could travel up to 2.5 hours, mileage reimbursed)
Required Qualifications for Patient Care Advocate:
  • Bachelor's degree Healthcare, Public Health, Nursing, psychology, Social Work, Health Administration
  • 4 years of total work experience required for background
  • 2+ years of recent experience in direct patient care, quality improvement, or social work setting
  • Understanding of HEDIS measures and quality improvement initiatives
  • HEDIS/HEDIS record review experience and Star Measure knowledge
  • Member outreach experience
  • Must live within Saginaw/Genesee counties (could travel up to 2.5 hours, mileage reimbursed)

Pay Details: $30/hour - $34/hour
