Title : Client Support Specialist
General Description
The Care Coordinator supports the individuals served at Victory Bay through coordination of their medical and
clinical treatment needs. Through collaboration with both the medical and clinical teams, the Care Coordinator
supports individuals to achieve their recovery goals.
Tasks & Responsibilities
1. Monitor tracking system of required UDS.
2. Obtain UDS from clients.
3. Liase with the laboratory.
4. Coordinate the sending of confirmation screens to lab.
5. Report positive screening results to the clinical and medical teams.
6. Enter UDS results into EHR system.
7. Support the coordination of external referrals.
8. Auditing Ancillary notes for completion
9. Facilitate Recovery oriented support groups.
B. Additional Duties
1. As the company continues to grow, additional responsibilities that are essential for the department to
be successful will be assigned accordingly.

Qualifications
Education:
• High School Diploma or GED
Experience:
• 1+ year professional experience in the behavioral health field; Lived experience can be accepted in
lieu of.
