Healthcare Social Worker

We are currently seeking a full-time Healthcare Social Worker (LMSW or LLMSW) to join our Skilled Homecare division.

Responsibilities:

• Conduct initial and ongoing psychosocial assessments
• Provide individual and family counseling
• Identify special needs related to religious or cultural diversity
• Evaluate financial or environmental concerns and direct patients/families to community resources
• Participate in discharge planning

Requirements:

• Master's degree in social work
• Active and in good standing Social Worker license (LMSW or LLMSW)
• Minimum 1 year of healthcare experience
• Hospice or homecare experience preferred
• Reliable transportation, valid driver's license, and car insurance
