We believe that everyone should have access to high-quality medical and dental care, regardless of ability to pay.Job purposeReporting to the Director of Behavioral Health, the Peer/Recovery Specialist is responsible for providing support to individuals with substance use and/or mental health concerns. This role coordinates with FHCHC clinicians and community organizations to identify and address the needs of clients, provide intervention and support services, and assist clients in achieving therapeutic goals. The ideal candidate has a deep understanding of recovery and harm reduction processes, is highly organized, and has excellent communication and interpersonal skills.Duties and responsibilitiesThe peer/Recovery Specialist will develop and implement individualized recovery plans for clients, including goals and objectives. Typical duties include but are not limited to:Practices a strengths-based approach to recovery and wellnessEngages peers in collaborative and caring relationshipsDocuments encounters in the electronic health record, as appropriateMonitors client progress and provide ongoing support and guidancePerforms standardized screening instruments with clients, including assessment of social determinants of healthConnects clients to appropriate community resources and servicesProvides crisis intervention and support servicesProvides education and resources to clients and familiesFacilitates weekly recovery groupsMaintains accurate records and documentationPrepares and submit reports to funding agencies, as neededCoordinates with other professionals to ensure continuity of careQualificationsLived experience with substance use, mental health concerns, homelessness, and/or sex workBilingual English/SpanishRecovery Support Specialist training and certification is preferredAt least one year experience working in a recovery-oriented settingDemonstrated knowledge of substance use, harm reduction, and mental health servicesExcellent communication and interpersonal skillsAble to work independently and as part of a teamProficient in Microsoft OfficeDirect ReportsNoneAmerican with Disabilities Requirements:External and internal applicants, as well as position incumbents who become disabled, must be able to perform the essential job specific functions (listed within each job specific responsibility) either unaided or with the assistance of a reasonable accommodation to be determined by the organization on a case by case basis.Fair Haven Community Health Care is an Equal Opportunity Employer.
