Positive Leaps is hiring a full-time Intake Therapist to join our team! The Intake Therapist will coordinate Positive Leaps intake and assessment activities to ensure quality of services to families and children.

  • Approximately 8 assessments per week with a small caseload of varying billable hours (up to 10 billable hours).

Essential Duties:
  • Collaborates with the Intake Specialist to coordinate and monitor potential clients through the referral and admission process.
  • Builds and maintains relationships with families, consumers, and the community throughout services.
  • Maintains work hours which are responsive to population served. May require occasional travel on-site to in-patient psychiatric unit of referral source.
  • Willingness to travel between locations to complete assessments as needed.
  • Ensures all required documentation is completed for consent to treat and identification of guardianship/custody.
  • Conducts mental health assessments to determine mental health diagnosis, and medical necessity for required level of care.
  • Communicates effectively with parents, peers, consumers, Team Members and supervisors in all departments to assist with admissions.
  • Prepares, updates and maintains documentation and records related to admission.
  • Ensures compliance with requirements of all funding sources relevant to admission, including pre-authorization for admission.
  • Provides clinical and case management services to families and children relative to admission proceedings.
  • In collaboration with the Program Coordinator, Therapy Services Coordinator and Billing Specialist, facilitates intake process such that admission to required level of care is timely, paperwork is accurate and complete, financial plan is complete, and consumer is satisfied.
  • Facilitates return of phone calls or communications as needed.
  • Creates an environment of enthusiasm and commitment.
  • Completes assigned tasks within assigned work hours.
  • Perform other duties as assigned, which can include but are not limited to maintaining a small caseload, assisting with coverage for colleagues, etc.
Minimum Requirements :
  • Master's degree from an accredited Social Work or Counseling Program.
  • Licensed by the State of Ohio as a social worker (LSW) or counselor (LPC) or having obtained independent licensure (LISW/LPCC).
  • At least two years' experience providing therapeutic mental health services and diagnosing children with mental health problems..
  • CPR and First Aid Certification.
  • Valid driver's license and auto insurance as travel between offices is required.
  • Excellent verbal and written communication skills.
  • Ability to work in a fast-paced work environment, including at least 9 assessment appointments per week.
  • Ability to run and carry up to 50 pounds unassisted.
  • Work schedule must be flexible as needed.
  • Ability to complete assigned tasks within assigned work hours and timelines.
Exposures and Risks:
  • Potential for exposure to blood and body fluids.
  • Works directly with physically aggressive children.
  • Exposed to loud noise, such as repeated and prolonged screaming and crying.
  • Expected to be able to run, work easily in a physically active environment and lift up to 50 pounds unassisted.
  • Fast paced, stressful environment.
