General Description
The Veterans Case Manager funded by the Texas Veterans Commission (TVC) will be responsible for providing comprehensive support and assistance to veterans in navigating various aspects of their lives post-service. This role involves assessing veterans' needs, coordinating resources, advocating on their behalf, and offering guidance to help them achieve stability and well-being. The ideal candidate will possess strong interpersonal skills, a deep understanding of veterans' challenges, and the ability to collaborate effectively with other service providers and community organizations.
Examples of Work Performed
Conducts a comprehensive assessment of veterans' needs such as housing, employment, financial stability, and social support.
Develops individualized care plans based on assessment and veterans' goals.
Connects veterans with appropriate resources and services, including healthcare providers, housing agencies, employment assistance programs, mental health professionals, and financial assistance programs.
Coordinates appointments and referrals to ensure timely access to services.
Advocates on behalf of veterans to ensure they receive the support and benefits needed.
Provides ongoing support and guidance to veterans as they navigate various systems and services.
Maintains accurate and up-to-date case files for each veteran, documenting assessments, care plans, progress notes, and referrals.
Monitors veterans' progress and adjust care plans as needed to address changing needs and goals.
Works with Director and Administrative Team for grant compliance.
Collaborates with other service providers, government agencies, and community organizations to ensure veterans have access to a comprehensive network of support.
Attends meetings, workshops, and training to stay informed about relevant resources and best practices.
Participates in outreach activities to raise awareness of available services and resources for veterans.
Cross train in all areas of the Social Service Program.
Performs all other duties as assigned by the Director.
Education and Experience
Bachelor's degree in social work, psychology, counseling, or a related field.
  • Two (2) years of related experience may be substituted for one (1) year of education.
One (1) year of related experience.
A Veteran or previous experience collaborating with veterans or military populations.
Certificates, Licenses, and Registration:
Must have a current valid Texas motor vehicle operator's license.
Must be able to be insured by the County's insurance carrier.
Knowledge, Skills and Abilities
Knowledge of veteran benefits and resources, including VA programs and services.
Strong communication and interpersonal skills.
Ability to work effectively both independently and as part of a team.
Ability to always maintain confidentiality and professionalism.
Ability to travel to various locations within the community to meet with veterans and attend meetings or training.
Knowledge of community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures.
Knowledge of effective group facilitation skills and ability to facilitate adolescent group/parenting counseling sessions.
Must have planning and organizational skills.
Skill in conducting inspections and interviews, in determining proper courses of action, and knowledge in using techniques, guidelines and standards to perform inspections.
Ability to prepare reports, interpret laws and regulations.
Proficient in the use of various software computer applications; Microsoft Word, Excel, Power Point.
Bilingual preferred.
