Qualifications

Position Summary

Responsible for the coordination of administrative and discharge planning for all Talbott Recovery patients

Essential Duties and Responsibilities

  • Establishes and maintains referral relationships.
  • Contacting referral sources upon admission and confirms patient's needs/expectations regarding updates/information.
  • Coordinates all communication and information exchange with referral.source.
  • Develops a comprehensive and patient-focused continuing care plan for assigned patients.
  • Timely and accurate input of contact information into established system.
  • Ability to access, assess and report on requested and/or needed information.

Education and Experience

  • Experienced in alcohol/substance abuse/12-Step program.
  • 1 to 2 years of such experience preferred
  • Must have a master's degree in related field
  • LCSW, LMSW, LPC, LAPC or Addiction Certification is preferred
