Talbott Recovery is currently looking for a fulltime Continuing Care Coordinator/Discharge Planner for our Main Campus on Hunter Road, Atlanta. QualificationsPosition Summary
Responsible for the coordination of administrative and discharge planning for all Talbott Recovery patients
Essential Duties and Responsibilities
- Establishes and maintains referral relationships.
- Contacting referral sources upon admission and confirms patient's needs/expectations regarding updates/information.
- Coordinates all communication and information exchange with referral.source.
- Develops a comprehensive and patient-focused continuing care plan for assigned patients.
- Timely and accurate input of contact information into established system.
- Ability to access, assess and report on requested and/or needed information.
Education and Experience
- Experienced in alcohol/substance abuse/12-Step program.
- 1 to 2 years of such experience preferred
- Must have a master's degree in related field
- LCSW, LMSW, LPC, LAPC or Addiction Certification is preferred
