Job Details

Home Care Field / Visiting RN seeing patients in their homes M-F 5x per week. Available territories are 1 - Queens. 2 Brooklyn (Pedi only), and 3 - Manhattan

What you will be doing:
· Complete initial assessments, OASIS, recertifications
· Creates and implements a plan of care
· Collaborates with the interdisciplinary team and coordinates patient care
· Educates patients and families

Skills you need:
· RN license in NY
· Home Care Experienced as a RN, LPN, CNA, or HHA preferred, but not required
