Job Summary

Provides clinical direction and education, works closely with operational and physician leaders, and has oversight of professional growth and development of their APRN's.

Essential Duties & Responsibilities

  1. Provides training for new NP's, monitors gaps in learning and oversees training plan for new NP's.
  2. Provides clinical direction and education for current NP's.
  3. Assists with professional growth and development of NP's.
  4. Provides assistance and feedback on NP performance for professional growth.
  5. Provides and establishes protocols/processes for Care Management - Cano at Home Program.
  6. Provides action plans for NP's to include productivity, documentation and efficacy of clinical care.
  7. Oversees the delivery of evidence-based practice care of NP's for chronic conditions and management to ensure that clinical standards are maintained.
  8. Works closely with operational and physician leaders.
  9. Sets goals for team and monitors progress.
  10. Reports and collaboratively works with Team Manager to identify areas of improvement with process and quality of care.

Supervisory Responsibilities

  1. Provides supervision and management of advanced practice providers (APRNs) in a distinct clinical service line - Cano at Home.

Education & Experience

  1. Master's Degree from a Nurse Practitioner or Physician Assistant program required, preferred specialties can include Internal Medicine, Adult Health, Community Health, Family Practice, Psychiatry.
  2. Two (2) or more years practicing experience as a Florida Board Certified Nurse Practitioner or Physician Assistant.
  3. At least one (1) year of experience in training, precepting, acting as a clinical advisor, providing unit support, working in the field independently/successfully.
  4. Preferred work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities.
  5. State of Florida Nurse Practitioner license.

Physical Requirements

This position works under usual office conditions. The employee is required to work at a personal computer as well as be on the phone for extended periods of time. Must be able to stand, sit, walk and occasionally climb. The incumbent must be able to work extended and flexible hours and weekends as needed.

Work Conditions

Must be able to perform essential functions such as typing, standing, sitting, stooping, and occasionally climbing.

Travel Requirements

Yes, 0-25% of the time. Work will involve traveling to patients' place of residence.

Tools & Equipment Used

Computer and peripherals, standard and customized software applications and tools, and usual office equipment.
