MedNOW Clinics is a growing integrated healthcare organization looking for an LPC or LCSW to work at our Lakewood clinic. We are innovative and inclusive. The position is for Integrated mental health therapy in a primary care setting with an emphasis on the social determinates of health.

Full Job Description:
  • Provide coping support, counseling services, and other appropriate support for patients during in-office or telehealth sessions
  • Document all communications with patients clearly and accurately with Athena Health electronic records system
  • Work with patients with multiple comorbidities including chronic health conditions, mental health concerns, social needs, and obstacles in their daily lives in an integrated care setting.
  • Provide continuous follow-up counseling sessions with patients to adhere to treatment plans and assess ongoing patient needs.
  • Demonstrate knowledge of differences between populations of differing ages, cultures, and home life situations – including psychological, physiological, and developmental differences between each group.
  • Assist patients in setting goals for their treatment plans
  • Ensure all patient information is kept confidential
  • Work alongside medical providers to develop and evaluate ongoing patient plans
