We are searching for a talented Community Outreach Specialist
Hours: 40hrs
Shift: Day/Evening shift with every other weekend rotation; 8:00am - 4:30pm
Location: Will consist of a hybrid schedule
Travel: Driving on average 3 or more times per week required

**Available to regular status employees who are scheduled to work a minimum of 24 hours.

Compensation: Pay rate will be determined based on level of experience.
Responsibilities
Position reports to the Manager, Community Benefits, or designee, in collaboration with a multidisciplinary care team build, leverage, and expand organizational support for equitable and accessible disease prevention and control among populations that are marginalized and disproportionately affected. Provide culturally appropriate health education, and outreach in a variety of settings including community-based, ambulatory, and acute care settings as needed. Implement culturally tailored interventions to meet patients short-term and long-term medical and associated health needs. Assist patients to practice healthy behaviours and access necessary social and medical support services and programs. Embed a health equity lens in all Community Wellness Program related activities.
Qualifications
  • Equal to completion of 4 years of high school plus additional specialized courses or training is required.
  • Bilingual skills required.
  • Two years experience in community-based outreach.
  • Proven ability for independent decision making and problem solving while performing effectively as part of a multi-disciplinary team.
  • Solid knowledge of community resources and effective teaching/coaching skills.
  • Verifiable excellent driving record.
  • Massachusetts Vehicle Operators Class B license is required.
  • A clear DMV driving record is also required.
  • Community Health Worker certificate preferred.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Pay Range
USD $21.62 - USD $36.36 /Hr.
