Position Summary: As a Home Health Physical Therapist, you will play a crucial role in helping patients regain their independence and improve their overall well-being. You will assess patients' physical conditions, develop personalized treatment plans, and provide therapeutic interventions to address various musculoskeletal, neurological, and cardiopulmonary conditions.
Responsibilities:
- Conduct comprehensive physical evaluations to assess patients' functional abilities and identify treatment goals.
- Develop and implement individualized treatment plans, including therapeutic exercises, modalities, and activities of daily living (ADLs) training.
- Monitor patients' progress and adjust treatment plans as needed.
- Educate patients and their families about self-care techniques and home exercise programs.
- Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure coordinated care.
- Maintain accurate and up-to-date patient records.
- Adhere to all company policies and procedures, as well as state and federal regulations.
Qualifications:
- Doctor of Physical Therapy (DPT) degree
- Arizona physical therapy license
- Minimum of 1 year of experience in home health or outpatient physical therapy preferred
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Passion for helping patients achieve their rehabilitation goals
