Position Summary
The Case Manager II supports the program by providing case management services supporting the emergency room at area hospitals as a liaison for referrals, linkage to various community mental health providers and crisis resources. The ultimate goal is to assist the patient with obtaining an income, insurance, crisis resources and other community resources. These services include assistance with co-occurring disorders (mental health and substance abuse) with the expectation of improving current functioning level and skills acquisition through assessment, engagement, assistance with access to treatment including medical care with a primary care provider, rehabilitation and goals.

Physical Demands and Work Environment
The work environment is characteristic of a typical office environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands and arms to reach. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.
The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.

Required Qualifications
- High School Diploma/GED (SUD Certification or AA/Bachelor's degree preferred) and 12 months Case Management experience with demonstrated ability to successfully perform duties.
- Must be able to represent WellSpace Health in a positive and professional manner.
- Must be able to work collaboratively with others in a team environment, respecting and valuing the perspectives and experiences of others.
- Twelve (12) months of experience with Harm Reduction, Motivational Interviewing, Patient Centered theory, Cultural Humility, in addition to Substance Abuse issues and Treatment.
- Able to maintain and understand appropriate roles, and boundaries among patients, peers, and WellSpace partners/stakeholders.
- Familiar with housing partners and community agencies referral and application processes.
- Able to function effectively in a fast paced environment.
- Ability to write progress notes and maintain detailed, statistical records and treatment plans.
- Demonstrate a commitment to the provision of services for the underserved.
- Able to rotate after hours on-call phone duties for a week at a time including weekends if needed.
- Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.
- Computer (PC) literate in Microsoft Word, Excel, Outlook email and database software.
- Current CPR and first aid certificate or willingness to be trained required within first (90) ninety days of employment.
- Must be willing to transport patients using personal vehicle, must have valid driver's license and meet commercial auto insurance policy requirements for acceptable driving record.
- Ability to abide by standards of professional ethics and maintain HIPAA rules of confidentiality.
- Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
- Assist and support the Center/Department/Program to meet standards of High Reliability.
- Meet with each patient individually and conduct an intake needs assessment.
- Attend rounds and multiple disaplinary team meetings.
- Represent and/or advocate for and with patients in obtaining identification, housing, benefits services, and entitlements from governmental and private agencies and organizations.
- Support patients with completion of HMIS, housing plan, housing resources, completion of housing application and move in process.
- Support patient with sustaining housing.
- Participate in care coordination meetings with various partners as deemed appropriate.
- Coordinate linkage to various community resource programs and supports.
- Collaborate with the patient in the development of their individual SMART treatment plan.
- Write and enter progress notes daily into EHR systems to maintain patient files.
- Maintain and participate in all job related training, certification renewals.
- Attend regular meetings with management and team.
- Connect patients with medical insurance and a Health Home Center.
- Refer and schedule patients with appointments with their health care providers.
- Provide transportation assistance and accompany the patient as needed to ensure patient accesses community resources and medical appointments.
- Keep accurate records for reporting to various funding and inspecting agencies.
- Willingness to accept a work location that could vary based on the needs of the program.
- Perform other duties as assigned.
