Job Responsibilities

Delivering high quality post-acute care in accordance with resident care plans, nursing best practices, facility policies, and all legal requirements

Coordinating care with the wider nursing and clinical team

Assisting RNs and clinicians with tests and medical procedures

Measuring and recording residents' vital signs and their physical and mental conditions

Administering medications and vaccinations as directed

Obtaining resident medical histories and ensuring that medical records are current, complete, and confidential

Listening to residents and communicating their concerns to staff

Engaging with residents and family members to foster trust, relationship building, comfortable living conditions, and resident satisfaction

Assisting with the training of nursing and non-nursing support staff

Promoting a safe and inclusive environment for residents and fellow staff members

Additional duties and responsibilities assigned by leadership team.

Qualifications and Skills

Active and unencumbered Ohio LPN license

Graduation from an accredited nursing program

Able to take the initiative as a self-starter and work independently and efficiently

Must have reliable transportation
