Position Description
The Care Coordinator will manage the planning process as well as provision of services and administrative functions for youth and families who are receiving services. The focus is on provisions of intensive service coordination and family support services to youth who meet the target population and eligibility requirements. The goal of Teen Violence Reduction Program is to enhance the family and youth's wellbeing by keeping the youth in the family home and community while promoting self-advocacy. Because of the life experiences and trauma histories that the youth and/or families bring when they become involved with the program, Care Coordinators must not only provide immediate safety/crisis planning skills but must be actively engaged in the process of building relationships, understanding, and solving problems as part of a team, and must offer energy, safety and commitment to their work through genuine, caring and empathetic engagement. The Care Coordinator will manage a caseload of up to 30 families to provide the necessary assistance identified by the family as well as the community and natural support providers.
The Care Coordinator assists participants in identifying and gaining access to medical, social, educational, and other services. The Care Coordinator encourages the use of community resources through referral to appropriate traditional and non-traditional providers, paid, unpaid, and natural supports. Care Coordinators work in partnership with the youth and their family/caregiver/legal guardian and are responsible for assembling the child and family team for a Child and Family Team Meeting (CFTM).
The Care Coordinator must be emotionally stable, able to handle high stress, relate well to a wide variety of individuals, and be able to work as part of a team. The Care Coordinator must be able to follow, enforce and implement program descriptions as outlined in policy and procedure.
The Care Coordinator must be able to provide one-to-one interaction with a designated child/adolescent and their family; act as liaisons and advocates for youth and families with the therapist and other team members or other agencies as needed, complete required paperwork, and perform other related duties as required. Care Coordinators must be flexible in working hours (available in the evenings after traditional work/school hours as to avoid disruption of their day), be able to respond to crisis situations in an effective manner and meet regularly with their supervisor.
This position provides care coordination to consumers/residents/member and the primary duties include but are not limited to the following:

This position typically works 10am -6pm Monday through Friday with occasional evenings and Saturdays to attend community events. (Time may vary based on needs for program and needs of the community) This position requires flexibility. The position also requires some light travel.

Role and Responsibilities
- Aid clients in connecting the client with resources to meet needs identified on needs assessment.
- Diligently track all referrals, forms, and outcomes as it relates to services/resources provided.
- Distribute, collect, and record all DYPI forms either face to face, at community events and through provider partners.
- Plan in a proactive manner to assist the client and/or family in connecting them to local services.
- Work collaboratively with representatives from partner organizations to ensure communication and service delivery is effective.
- Provide service and resource coordination to assist the client and/or family in gaining access to necessary rehabilitative, medical, social, and other services and supports (E.g., Medicaid, SSI, consents, etc.)
- Assist clients with making appointments for services that meet their needs.
- Provide follow-up contact with contact following their scheduled appointments. Solicit feedback regarding their appointment. Track all feedback and surveys.
- Assure clients are actively contacted and encouraged to participate in services.
- Collaborate 100% with other agency staff to accomplish work related activities.
- Document services and contacts with each in an excel spreadsheet or in EHR.
- Maintain a positive attitude toward all clients and their families 100% of the time.
- Attend and participate in scheduled staff meetings, administrative meetings, and in-service trainings 100% of the time.
- Abide by all rules of confidentiality and the Code of Ethics prescribed by CHRIS 180 100% of the time.
- Positive attitude, attention to detail and phone skills needed.
- Will work closely with the other clinical staff at CHRIS 180 including Therapists, Community Behavior Specialists, and others as indicated.
