Job Requirements / Description

Under the direction of a case management coordinator, assesses clients' case management needs and goals, treatment plans with client to address needs and goal, refers and links clients to community services and resources reflective of plan, and monitors service delivery and evaluates effectiveness of resource/service provisions. Bachelor's degree from an accredited four-year college or university.

EDUCATIONAL REQUIREMENTS
  • Bachelor's degree from an accredited four-year college or university.
QUALIFICATIONS
  • Maintains auto insurance as acceptable by minimum standards; in addition, names Mojave as an additional insured/interested party and maintains "business use" on policy during course of employment.
  • Automobile in good working condition, in order to transport clients if needed
  • Current and valid driver's license.
  • Clear driving record. A motor vehicle report will be ran.
  • In accordance with the conditional offer of employment, individual must pass drug screen and background investigation
PYSICAL REQUIREMENTS
  • May include standing, sitting, and/or walking for extended periods
  • May include performing repetitive tasks
  • May include working on a special schedule (i.e. evenings and weekends with clients)
  • May include working with challenging patients and clients
