Coordinate all aspects of enrollment and disenrollment.Liaise with appropriate health and related entities; including but not limited to hospitals, home care, and hospice as well as the broader community.Contribute to accurate reporting of program enrollment and relevant statistics.Perform communication and outreach activities.
