Responsibilities:

• Provide support and assistance to individuals in need, focusing on social work, behavioral health, addiction counseling, crisis management, crisis intervention, mental health counseling, motivational interviewing, systems, and mentoring.
• Conduct peer group and assessments to determine the needs and goals of individuals seeking support.
• Develop and implement personalized care plans to address the unique needs of each individual.
• Collaborate with other professionals and agencies to coordinate services and resources for clients.
• Offer counseling and guidance to individuals facing various challenges in their lives.
• Assist individuals in accessing community resources and support networks.
• Advocate for clients' rights and ensure they receive appropriate care and services.
• Maintain accurate records of client interactions and progress.

Skills:

• Strong background in social work, behavioral health, addiction counseling, crisis management, crisis intervention, mental health counseling, motivational interviewing, systems, and mentoring.
• Excellent communication skills, both verbal and written.
• Empathy and compassion for individuals facing difficult situations.
• Ability to assess needs and develop effective care plans.
• Strong problem-solving skills to address complex issues.
• Knowledge of community resources and support networks.
• Ability to work collaboratively with other professionals and agencies.
• Strong organizational skills to maintain accurate records.
