Under the general supervision of the agency Clinical Services Manager, is responsible for overseeing the health care of all consumers served by DSA, Inc. as well as providing direct nursing services to a group of consumers. In all her/his endeavors, shall promote the agency mission statement to maximize the quality of life experiences for our consumers; inspire the confidence of the families of our consumers, provide growth opportunities and economic stability for all DSA staff and participate in shaping local, state and national policies affecting persons with disabilities. Responsible for ensuring that program services are appropriately rendered to meet the medical needs of consumers consistent with DSA, Inc. policies and procedures and applicable state and federal regulations. Oversees the agency health care systems to include medication administration, availability of appropriate allied service providers, and training programs for staff to meet the needs of consumers.
Responsibilities and Duties
  • Provides direct and individualized nursing care as assigned to a caseload of consumers to include consumers receiving Wellness Coordination Services.
  • Responsible for any Wellness Coordination Services on assigned caseload to include appropriate documentation and service provision as required by State regulations.
  • Ensures the prescribed medications and treatments are available in the home for each consumer.
  • Ensures the development, documenting, implementing, and evaluating, for each consumer, health care protocols and nursing care as required by the Individual Support Plan (ISP).
  • Acts as a resource for agency LPNs as assigned.
  • Ensures the monitoring of vital signs, weight charts, immunization records, and health status of DSA consumers.
  • Routinely reviews the agency medication error report and makes recommendations to the Executive Council.
  • Responsible for managing the agency Psychotropic Medication Review Clinic as assigned.
  • Participates in Area Individual Support Team (IST) meetings, sharing knowledge, information, and ideas among team members.
  • Records information concisely, accurately, and completely.
  • Ensures consumer's medical information chart is current.
  • Maintains a positive working relationship with family members and guardians of DSA consumers.
  • Serves as the primary contact when a consumer is hospitalized. Responsible for communicating information between the agency staff and hospital staff.
  • Assists in the development of staff training curriculums, including training in detecting signs of illness or dysfunction that warrant medical or nursing intervention, basic skills required to meet the health needs of the consumers, Core A & B curriculum, CPR, and first-aid in the presence of an accident or illness.
  • Administers and reads TB skin tests as necessary for consumers and staff.
  • Ensures nursing assessments are completed per agency policy.
  • Serves as a liaison between health care allied service providers and the Area Directors.
  • Ensures consumer medical records are maintained in a manner consistent with HIPAA regulations, other Federal and State regulations, and applicable DSA, Inc. policies and procedures.
  • Participates in the nursing on-call schedule
  • Serves as a DSA, Inc. representative to trade organizations and state or regional committees.
  • Participates in routine supervisory reviews with the Clinical Services Manager
  • Appoints a person to act on her/his behalf during absences
  • Other duties as assigned

Qualifications and Skills
  • Degree from an accredited school of nursing.
  • Current state licensure as a licensed nurse.
  • Five years of clinical practice preferred.
  • BSN preferred.
  • One year of experience working with individuals with developmental disabilities or in a related field preferred.
  • Maintains credentials necessary to teach Core A/B courses.
  • Maintains certification necessary to administer and read tuberculosis skin tests.
  • Must be at least twenty-one (21) years of age.
  • Ability to communicate effectively; orally and in writing.
  • Ability to use common word-processing, spreadsheet, and communications software programs within a Windows environment.
  • Satisfactory completion of criminal background checks as required by federal and/or state laws and any other post-offer pre-employment screenings as mandated by a governing agency or DSA, Inc.
  • Valid Indiana driver's license, a good driving record and a current automobile liability insurance policy are required.
