Summary of Essential Duties:
- Serve as a clinical co-pilot—charting the best trajectory of each patient’s care by accommodating various needs, including medical, psycho-social, financial and practical.
- Coordinate the care and service of the patient population from admission through discharge.
- Assess and plan, implement, monitor, and evaluate options and services to meet an individual's health needs to facilitate a safe and timely discharge to the next appropriate level of care.
Minimum Requirements:
- Associates Degree in Nursing required, CA License required
- Experience and certification in Case Management preferred
- Case Management certification expected within 2 years of hire
- BLS Certified
- 3 years minimum in Acute Care Nursing
