We are seeking a Homecare Social Worker to provide personalized care to patients, collaborate with the interdisciplinary team, complete comprehensive in-home psychosocial evaluations, provide clinical treatment services, offer education to patients and families, advocate for patient needs, and maintain timely documentation of social work services. The ideal candidate must have 2-3 years of healthcare experience and hold an LMSW license.
Qualifications
- 2-3 years health care experience, inclusive of acute and community health experience
Licensure, Certification, Registration
- LMSW
Knowledge, Skills and Ability Requirements
- Considerable knowledge of social, cultural, economic, political, religious, medical, psychological and legal issues influencing patient behaviors
- Familiarity with various evidence-based assessment tools
- Strong communication skills and computer proficiency
