Case Manager - Integration

OVERVIEW
The Case Manager for the Integration Program will provide support to recently arrived refugees via long-term case management beyond the initial resettlement period to aid clients in achieving all aspects of self-sufficiency, including health and medical assistance, employment, and English language instruction. The manager will also work collaboratively alongside partnering community agencies, make referrals for services and resources, and provide transportation to clients as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide services to clients as outlined in the NYSESRP contract guidelines, including case management, health and medical assistance, ESL training assistance, and employment assistance.
  • Work together with clients to develop a Service Plan and set realistic goals
  • Create and send referrals to partnering external service providers through online platforms to remove barriers to self-sufficiency.
  • Maintain case files and relevant databases as outlined within the NYSESERP contract.
  • Maintain internal and up to date case note logs for client services through USCRI Sharepoint Intranet.
  • Work with Employment team to support clients in obtaining job training, full time employment, and advanced careers.
  • Establish connections for clients and provide orientation/guidance to access them across local networks within cultural groups and the community at large.
  • Provide linguistic and culturally appropriate services and advocate for language accessibility in public settings like medical offices.
  • Perform other job-related duties as assigned by supervisor.

POSITION REQUIREMENTS
  • High School diploma or GED required; Bachelor's degree from an accredited institution is preferred.
  • Experience in case management, writing case notes, human services, or social work preferred.
  • Exceptional interpersonal skills, with the ability to work across diverse populations, a keen listening ability, and relationship building skills required.
  • Capacity to work as a team member and independently; excellent organization and time management skills required.
  • Ability to work efficiently in a fast-paced environment, troubleshoot, and follow projects through without loss of attention to detail.
  • Experience with refugee populations preferred.
  • Excellent communication skills with proficient written command of English.
  • Proficiency in Microsoft Office and basic technology skills.
  • Secondary language skills are not required, but preferred; particularly Arabic, Pashto, Dari, Urdu, Farsi, Spanish, Ukranian, Burmese, Karen, and Swahili.
  • Must pass a criminal background and other screening requirements.
  • Must have a valid driver's license, insurance, clean driving record and personal transportation, and be able to transport clients as needed.

TRAINING REQUIREMENTS
  • Satisfactory Completion of USCRI's Orientation and Training.
  • Completion of NYSESRP BIN Training through NYS ODTA.
  • Complete additional training as identified by supervisor(s) or Human Resources.
PHYSICAL DEMANDS
  • Use of manual dexterity, tactile, visual, and audio acuity.
  • Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands.
  • Occasional lifting (up to 25 pounds), bending, pulling, and carrying.
  • Quantitative/mathematical ability (addition, subtraction, multiplication, division, standard measurements).
