Join our team at Christ Health Center as a Mental Health Services Coordinator based out of Live Oaks, our Behavioral Health Clinic. We are seeking an individual to coordinate mental health services for patients with behavioral health disorders under the direction of our staff psychiatrist. The social services coordinator will maintain ongoing contact between the patient and the provider under the provider's direction to facilitate treatment and care for patients needing behavioral health care. This position is primarily administrative with very minimal in-person patient interaction. The Mental Health Services coordinator will gather mental health history, medication history, and referrals to facilitate scheduling and patient care. Major Duties & Responsibilities: Job Skills

  • Assists in completing disability forms and other medical forms as requested by the patient.
  • Administer routine medications as ordered by the provider (RN only).
  • Assists with recognizing the needs and concerns of people within their cultural framework, and maintains constructive relationships in dealing with Live Oaks patients.
  • Responsible for ordering, maintaining and storage of laboratory supplies and equipment.
  • Draw laboratory tests as ordered by the provider and report test results to the patient as directed.
  • Uses Telephone Triage Protocols and Crisis Intervention Protocols as guides.
  • Requests refills on routine medications and responds to requests for refills from pharmacies.
  • Understands CHC's relationship with the community and maintains a positive image and relationships
  • Attends and participates in in-service education for staff, quality assurance program, and risk-management programs.
  • Each employee of Christ Health Center is vital to providing overall quality care to our patients and may be included in the patient care team and daily huddle when deemed necessary by the Core Care Team.
  • Primarily involves administrative tasks, such as communicating with patients via telephone and the patient portal.
  • Performs other duties as assigned.
Requirements
Required Skills/Abilities
  • Excellent verbal and written communications skills.
  • Requires effective interpersonal skills
  • Requires analytical skills, attention to detail, effective organization skills, ability to work in a fast-paced environment and ability to self-direct with minimal supervision.
  • Requires the ability to work in a multi-cultural setting (Bi-Lingual in Spanish is preferred)
  • Proficient in Microsoft Office (Word, Excel, Outlook), use of EHR.
Qualification, Education, Experience
  • Bachelors or master's in social work or nursing license.
  • Current CPR certification preferred
  • 1 year psychiatric or behavioral health experience is preferred
Physical/Mental Demands
  • Work requires the ability to lift objects weighing up to 20 pounds
  • Work requires ability to carry objects weighing up to 20 pounds.
  • Work requires ability to sit +/- 90% of the time.
  • Work requires ability to stoop and bend, to reach, and grab with arms and hands; manual dexterity and to communicate with others.
  • Work requires proofreading and checking documents for accuracy.
  • Work requires ability to use a keyboard and visualize a computer monitor to enter and transform words or data. Specific vision abilities required include close vision and the ability to adjust focus
  • OSHA personal exposure risk category I & II
  • Airborne personal exposure risk category I (Possible exposure of the healthcare worker to patients capable of transmitting M. tuberculosis). Universal precautions will be adhered to at all times.
Language Skills
Ability to read and comprehend simple instructions, short correspondence, and memos; ability to write simple correspondence and reports; Ability to effectively present information in one-on-one and small group situations to patients, visitors, and other employees of the organization.
Mathematical Skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals; ability to compute rate, ratio, and percent.
Reasoning Ability
The ability to apply general rules to specific problems to produce answers that make sense. The ability to combine pieces of information to form general rules or conclusions (includes finding a relationship among seemingly unrelated events) and to convey the information to others.
