Overview:

Cal-AIM Housing Navigators are crucial in advancing the agency’s mission and vision. They ensure members authorized by Managed Care Plans (MCPs) receive timely and quality housing transitional navigation services, including housing deposits, tenancy, and sustaining services. They also deliver Community Supports housing services and collaborate with local Continuum of Care (CoC) homeless service systems, MCPs, and Enhanced Care Management (ECM) providers. Reporting to the Cal-AIM Program Manager, Housing Navigators must have experience in housing and homelessness and a solid understanding of HUD regulations. Strong communication and organizational skills are essential.


Responsibilities and Duties:


  • Collaborate with team members for outreach strategies targeting isolated consumers.
  • Commit to consumer autonomy and empowerment.
  • Build respectful interpersonal relationships with consumers.
  • Support consumers in identifying barriers and finding appropriate healthcare resources.
  • Assist members in accessing education and employment services.
  • Increase awareness of self-help programs and consumer advocacy organizations.
  • Encourage members to leverage their talents and skills.
  • Participate in community presentations about the Cal-AIM program.
  • Attend staff meetings for program development and training.
  • Exhibit non-judgmental listening with respectful engagement.
  • Maintain punctuality and regular attendance.
  • Model agency mission and core values in all communications.
  • Communicate effectively in a culturally competent manner.
  • Promote positive interactions among managers and colleagues.
  • Provide caregivers with information about the agency’s processes and team members.
  • Engage in continuous learning for best consumer engagement practices.
  • Complete intakes and assessments to determine housing needs.
  • Identify members for HMIS CES Surveys.
  • Meet with members weekly to monitor housing progress.
  • Develop individualized housing support plans.
  • Secure resources for affordable housing and rental subsidies.
  • Support members in compiling documents and filling out housing applications.
  • Conduct pre-inspections to ensure units meet Housing Quality Standards (HQS).
  • Guide members in requesting security deposit assistance and managing fees.
  • Assist with obtaining household goods for safety and accessibility.
  • Facilitate landlord/tenant mediation and educate clients on rights.
  • Maintain documentation in compliance with program contracts and agency standards.
  • Conduct health & safety home visits and habitability inspections.
  • Address potential behaviors jeopardizing housing, such as late payments or hoarding.
  • Collaborate with community resources to prevent eviction.
  • Assist with benefits advocacy, including SSI application support.

Qualifications:


Competence in performing each essential duty is required. Accommodations may be made for disabilities.


Minimum Education and/or Experience:


Bachelor’s degree or related experience in a social service field, or 3 years of experience in case management, housing supportive services, and working with the homeless population.

Knowledge of housing rights and regulations for low- and moderate-income housing.


Other Specific Requirements:


Valid California driver’s license and insurable driving record if driving is required. If not driving, the ability to use public transportation.

Extensive knowledge of local community resources.
