Description
The Community Health Representative serves to enhance and protect the quality of life for tribal members within the service delivery area, through outreach to Health Clinic patients and clients in order to maximize the effective utilization of available health care, health education, disease prevention dental, mental health and social services. The position operates under the direct supervision of the CHR Supervisor and the guidance of the Health Clinic RN.
Duties Include:
  • Educating: Assists individuals and families in the timely, effective and appropriate use of health facilities, resources and preventative and curative practices and services. Informs, community members of the services available at the SKlallam Health Center and other resources in our area, provides appropriate informative materials.
  • Home/Hospital Visits: Provides home visits for community members who are hospitalized, physically or mentally disabled or homebound, as directed by Tribal Health Clinic staff and/or outside medical providers.
  • Resource Access Assistance: Assists individuals interpreting eligibility requirements, completing necessary forms and applications and problem solving with various health and social services agencies and providers.
  • Patient Treatment and Self-Care: Assists clients and patients with the proper use of medication and implementation of treatment and self-care plans as directed by Health Services and outside medical providers. Provides consistent ongoing contact with patients with chronic health problems and/or confined to nursing or extended care facilities. Assists elders in the implementation of exercise, walking and stretching routines outlined by health care providers.
  • Medication distribution: Pick up and deliver all prescriptions to eligible community members, in accordance with established policies and procedures (elders, disabled) under the guidance of the Health Clinic RN and Community Health Nurse.
  • Transporting, Advocacy and Liaison: Transports clients to health care facilities as necessary, in accordance with established policies and procedures. Accompanies clients to health providers, facilitates communication and acts as patient advocate.
  • Paratransit: Responsible for utilization of the Paratransit Services contract. This includes tracking client eligibility, completion and submission of billing forms, and participation with agency audits.
  • Meetings: Attends and participates in various community meetings and staff committees whose goals directly or indirectly relate to the SKlallam Community Health Representative program objectives.
  • Health Fairs and Other Community Events: Coordinates and prepares information and topics for community health fairs and other types of tribal events focused on health education, exercise and other disease prevention activities.
  • Personal Contacts: Maintains regular contacts of a positive nature with elders, staff, and community members and as effective liaison with Indian Health Service, DSHS, KC Health District, Tribal Dental Clinic, local physicians and medical service providers, local fire department staff, and others to accomplish SKlallam Health Clinic objectives and preserve good public relations.
  • Reporting: Produces and distributes written and/or verbal reports to Health Clinic staff and outside providers in a timely manner as directed.
Qualifications:
  • Education: High School Diploma or GED required.
  • Licensing:, First Aid Certification. Defensive Driving Certification are required.
  • Other: Basic computer skills and the ability to learn word-processing, spreadsheet and other software programs are required
  • Valid Washington Drivers license and acceptable 3 year driving history.
Other Requirements :
  • All Health Services Department staff members who provide client transportation services, whether full-time, part-time, on-call or volunteer, may be subject to annual drug testing.
  • Provide transport for clients and patients in tribally-owned vehicles to health care appointments and other transports as assigned and referring any inappropriate client and/or patient to the CHR Supervisor for resolution.
Travel Requirements:
  • Local, statewide and out of state travel may be required.

  • Occasionally required to lift and/or move up to 50 pounds
