Starting Wage Range: $22.36/hr. - $29.00/hr.
Job Summary:
Promotes holistic health and wellness practices that enable clients to practice self-care techniques in order to make healthy lifestyle decisions and improve their overall quality of life.
Essential Functions:
  • Provides client services that promote holistic health and wellness practices.
  • Conducts and plans various family and youth activities in collaboration with Activities Wellness Promotion Coordinator.
  • Participates in regularly scheduled preschool wellness activities.
  • Conducts home visits performing health screening of clients with chronic, acute, and communicable illnesses.
  • Provides follow-up in-home treatments, monitoring client's health conditions and vital signs as delegated by medical providers or licensed nursing staff.
  • Participates in regularly scheduled case management meetings.
  • Provides support to Diabetes support group.
  • Assists with the set up and maintenance of Medical Alert system, providing ongoing support to clients.
  • Participates in mobile clinic events.
  • Transports clients to medical appointments.
  • Adheres to HIPAA law as it impacts a client's private health information.
  • Maintains accurate and current client records and program reporting as required, including documentation in electronic health record and shared department folders.
  • Performs a variety of other duties as assigned by supervisor.
Education, Experience, and Licenses:
  • Requires a High School Diploma or GED and a minimum of two (2) years of post-high school/GED specialized trade or job skills training in health and wellness such as coursework in Certified Nurse's Assistant training or Community Health Representative
  • OR equivalent combination of education, training and experience.
Driving Requirements:
  • This position may require driving.
Qualifications, Registrations, and Certifications :
  • Completion of Community Health Representative training modules.
  • Requires First Aid and CPR certification.
  • Requires knowledge of disease prevention and basic caregiver techniques. Native American cultures and geriatrics are examples of course work acquired at this level.
  • Requires demonstrated experience with performing health screenings to include operation of blood pressure monitors, thermometers, and blood glucose machines.
  • Requires demonstrated knowledge of Microsoft products, Nextgen electronic health record, copy and fax machine, and telephone system
  • Must have working knowledge of HIPAA law.
  • Knowledge of and sensitivity to Native American culture required. Native American employees preferred.
Environmental Factors and Conditions/Physical Requirements:
  • Continually requires the ability to express or receive detailed information or important instructions through oral communication.
  • Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and/or fingers. Often requires walking or moving about to accomplish tasks.
  • Requires standing and/or sitting for sustained periods of time, raising objects from a lower to a higher position and/or moving objects horizontally.
  • Occasionally requires lifting of up to 40 pounds.
  • Must have the ability to work at times with minimal supervision.
  • Due to the nature of the interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations.
  • This position is frequently subject to inside environmental conditions which provide protection from weather conditions, but not necessarily from temperature changes, and is occasionally subject to outside environmental conditions.
  • May be called upon to serve as a Disaster Reservist during emergencies.
  • Local travel is occasionally required. Travel outside of the state is infrequently required.
