Job Description:Benefits EligibleScheduled Weekly Hours: Monday - Thursday (40 hours) 7:00am-5:30pmNo weekends/holidaysAdditional Details: Position requires to be on-site at St. Mary’s Integrated Addiction Medicine Clinic (Outpatient)The Care Coordinator is a patient-focused role that helps successfully manage the comprehensive care of patients. This position provides customer service, proactive outreach to patients, and administrative support to clinicians and care teams. The Care Coordinator is responsible for managing inbound and outbound calls to schedule appointments, utilizing analytics to help close gaps in care, supporting patients to meet their goals, coordinating resources to help patients overcome socioeconomic barriers, and resolving patient issues when possible. This includes receiving, prioritizing, documenting, and actively resolving caregiver requests. This position reports to a Care Coordination Supervisor and works collaboratively with the Care Coordination Manager, Operations Transformation, Network Management, Care Management, Providers, and various members of clinic staff.ScopeAs a non-clinical Care Coordinator you need to know how toDemonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients servedCollaborate with the health care team to develop a plan of care by participating in patient careDemonstrate knowledge of the principles of growth and development over the life span and possess the ability to differentiate the required careHelp facilitate the safe care of patients, including facilitating the appropriate exchange of information among all caregiversIdentify patients requiring care management and take the lead as Care Coordinator for those requiring intervention, especially with socially complex casesParticipate in co-visits with providers using a multidisciplinary approach as neededServe as support to SCL Health for population management, patient self-management, along with support and tracking and intervening with high utilizers and those identified as medically at riskInterview patients and families to assess current strategies for self-care and available support systems to manage their healthPartner with community resources and community referrals sources as well as build new relationship with the direct community for patient care needsDocument correctly and accuratelyParticipate in clinic and team Quality Improvement processes.Minimum QualificationsAssociates DegreeCurrent BLS certification endorsed by the American Heart Association-Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals-Knowledge and experience with managed care-Minimum of three (3) years of relevant experience and a minimum of two (2) years of previous formal education activitiesPreferredBachelors DegreeAt least three (3) years of healthcare or social services experiencePhysical Requirements:Physical RequirementsInteract with others requiring employee to verbally communicate as well as hear and understand spoken information.Operate computers, telephones, office equipment, including manipulating paper requiring the ability to move fingers and hands.See and read computer monitors and documents.Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.Location:Integrated Addiction ClinicWork City:Grand JunctionWork State:ColoradoScheduled Weekly Hours:40The hourly range for this position is listed below.
